  1. Log in to your myClubhouse site.

  2. Navigate to “Forms” Forms > “Forms Manager”Forms Manager.

  3. Click “Add new”.

  4. Enter the Name for the Form.

  5. Enter a short description for the Form, including an outline of the information it is gathering.

  6. Leave “Form refers to” as “User”.

  7. Leave “Visible to User” and “Editable by User” checked.

  8. Check “Editable by User after completion”. This will allow Users to update their Emergency Contact details in the future.

  9. Leave “Restrict to Member List” set to “No Restriction” so that all registered Users can set their Emergency Contact details.

  10. Enter some introductory text. This will appear at the top of the Form when it is being completed.

  11. Click “Add Question”.

  12. Enter the question text. Here this will be the name of the Emergency Contact.

  13. Select “Text” as the answer type.

  14. Enter the short name to identify the question.

  15. Click the asterisk to make the question compulsory.

  16. Click “Add Question”.

  17. Enter the question text. Here this will be the number for the Emergency Contact.

  18. Select “Text” as the answer type.

  19. Enter the short name to identify the question.

  20. Click the asterisk to make the question compulsory.

  21. Click “Save & Close”.

  22. Navigate to “Forms” Forms > “Set Set up Attribute Questions”Questions.

  23. Click “Add”.

  24. Select the Form you just created.

  25. Select the emergency contact name question.

  26. Modify the Code to identify the Attribute.

  27. Modify the Caption for the Attribute.

  28. Enter a Description for the Attribute.

  29. Leave “Show in Profile?” set to “No”.

  30. Click “OK”.

  31. Repeat steps 23-29 to add the emergency contact number question.

  32. You now have Attributes to hold your members' emergency contact information. These can be accessed on a user's profile, shown in the Member Directory, used as criteria in Member Lists, and as filters within parts of your myClubhouse site.
